AMC MPHW Document Verification:-As per the approval received from the competent authority and Advertisement No. 22/2023-24 dated 04/09/2023, candidates who applied for the post of Multi-Purpose Health Worker (MPHW) are hereby informed to remain present for original document verification/scrutiny.
🗂️ The verification will be conducted at the following venue and schedule mentioned against each candidate’s name:
📍 Exhibition Hall, Ground Floor, Riverfront House, Sabarmati Riverfront (West), Ahmedabad
📌 Required Documents (Bring Originals + One Xerox Set)
Candidates must report with the following original documents and 1 set of Xerox copies:
✅ Valid Photo ID Proof
✅ School Leaving Certificate
✅ Educational Qualifications (Semester-wise mark sheets and degree certificate)
✅ Experience Certificates (if applicable)
✅ Caste Certificate (if applicable)
✅ Application Receipt
✅ Call Letter
🚫 Note: If any candidate is absent during the document verification, they will be disqualified from the further recruitment process.
